Overview
Social Worker – Medical Social Worker position in Everett, WA, starting 06/01/2026 for a 13-week travel assignment. Weekly pay range: $1,319–$1,390 per week. Guaranteed hours: 72 hours per 2 weeks. Shift: 4×8-hour days (days). Setting: hospital-level II trauma center with two adjacent campuses totaling 571 beds, serving adult and geriatric patients. The role includes cross-campus coverage within the local campus system and collaboration with an interdisciplinary team that includes PT, Pharmacy, Transportation, Interpretation Services, and Social Services. EPIC is the charting system used.

Responsibilities
- Conduct biopsychosocial assessments and provide discharge planning and care coordination for adult and geriatric patients.
- Facilitate referrals to home health, rehabilitation, long-term care, and community resources; support guardianship and POA arrangements as needed.
- Collaborate with interdisciplinary teams (PT, Pharmacy, Transportation, Interpretation Services, Social Services) to optimize patient flow and timely discharge.
- Manage a patient caseload with emphasis on discharge planning, care coordination, and appropriate resource utilization; document care plans in EPIC.
- Support patients and families in understanding treatment options, goals of care, and Medicare/Medicaid resources; participate in care conferences and rounds.
- Perform cross-campus coverage as required, including weekend duties and holidays per schedule, while maintaining compliance with hospital policies and patient privacy regulations.

Requirements
- Education: Master’s degree in Social Work (MSW) or current RN with relevant clinical social work scope; LMSW or LCSW with Master’s degree accepted per client guidelines.
- Licensure: Active Washington state licensure in the appropriate profession (MSW/LMSW/LCSW or RN); in-hand licensure preferred with eligibility noted for pending licenses per client policy.
- Certification: BLS certification required.
- Experience: Minimum 1 year of relevant experience (acute care discharge planning or care coordination); internal guidance may require 2 years—confirm client-specific requirements.
- Skills: EPIC charting proficiency; strong discharge planning and care coordination abilities; case management experience across Med-Surg, long LOS, and ED is highly preferred.
- Other: Ability to work in a Level II trauma center setting with cross-campus responsibilities; travel/commute as a traveler; weekend and holiday availability required (every other weekend and 3 of 5 major holidays); orientation and pre-start requirements completed prior to start.
