Job Title: Licensed Social Worker (LSW / CSW)Location: New JerseyJob Type: Full-TimeSetting: Adult Day Care Facility About the Role ATC Healthcare Services is seeking a qualified and compassionate Licensed Social Worker (LSW/CSW) to join our team in an Adult Day Care setting. In this role, you will be responsible for supporting participants’ emotional and social well-being, coordinating care services, and working closely with families and interdisciplinary teams to ensure high-quality, person-centered care in compliance with state regulations. Key Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care/service plans.
• Provide counseling, emotional support, and crisis intervention when required.
• Collaborate with healthcare providers, caregivers, and community resources to ensure continuity of care.
• Assist participants and families with accessing social services, benefits, and community-based programs.
• Maintain accurate, timely, and compliant documentation of all case records.
• Participate in interdisciplinary team meetings and contribute to care planning.
• Advocate for participants’ rights, dignity, and overall quality of life.
• Ensure strict adherence to HIPAA guidelines and all applicable state and federal regulations.

Qualifications

• Active New Jersey LSW or CSW license (required).
• Minimum of 3 years of experience in social work, healthcare, senior care, or community-based services preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to work effectively within a multidisciplinary team environment.
• Experience working with older adults or special needs populations is an advantage.
